University School Athletics Eligibility Policy

Initial Athletics Eligibility at the beginning of the school year will follow TSSAA guidelines. No student can begin a clinic, or tryouts without a Physical Form and Concussion Form on file with the school, for more information click on Athletics Eligibility Policy & PRE-PARTICIPATION form

University School Sports Physicals. All students who plan on participating in athletics must have a current physical, concussion, and cardiac arrest form on file with the school before they can participate in University School athletic programs:

• Physical Forms
• Cardiac Arrest Forms
• Concussion Forms

All must be completed and on file with the school in order for the athletes to practice. Parents mayscanand submit therequired forms tothis secure ETSU DropboxLink.

Coaches must also complete:

• Coaches - Concusion Form
• Coaches - Cardiac Arrest Form
• NFHS

**Reminder: All athletes must have an athletic physical , concussion form , and cardiac arrest form each year. Forms must be completed and on file with the school in order for the athletes to practice.
