Want to give a shout out to recognize a Quillen student, faculty or staff member for going the extra mile? Use the Quillen kudos form to recognize somone for going above and beyond! They will be recognized in the newsletter with a kudo and be part of a lunch for two monthly drawings.

CARE Forms
CARE forms are an online form that anyone associated with Quillen can complete to call attention to concerns regarding a student. Any concern about a student’s welfare may be reported. This includes medical, mental health, academic and physical health issues.
CARE Forms should not be used in an EMERGENCY situation or if immediate attention is needed. In case of Emergency, please dial 911.
CARE reports can be confidential, but you are encouraged to provide information in case additional information is needed. A limited number of student affairs staff have access to the form.
The reporter’s identity will not be disclosed to the student of concern.
CARE reports are reviewed daily by student affairs staff. If it is determined that further action is needed, the student will be contacted and interviewed in person to assure safety. Depending on the severity of the concern, additional steps will be taken based on the college's policy to include Fitness for Duty.

Concerns or Complaints
Quillen College of Medicine is committed to upholding a safe, respectful, inclusive, and effective learning environment, free of mistreatment, discrimination, humiliation or harassment.
Such a learning environment is essential to learning and promotes the ability of all students, residents, fellows, faculty, and healthcare and administrative staff to work together productively and collaboratively.
This form is used to report concerns about Quillen medical student, graduate student, resident, faculty, fellow, and staff mistreatment. Reports will go to the College of Medicine Grievance Officer and will be reviewed at least weekly. -

The Quillen College of Medicine fully supports ETSU’s values statement:

The Quillen College of Medicine is dedicated to helping future physicians develop and maintain professional behaviors. Professionalism may encompass communications between patients and providers, appropriate attire/appearance, reliability, motivation and responsibility, interpersonal relationships, ability to accept instruction and feedback and integrity.
The Quillen College of Medicine professionalism policy and procedures support the ongoing development of learners and provide a framework for reporting unprofessional behaviors and correcting any deficiencies.
The Student Professionalism Report is designed to allow faculty, staff or students to document an incident or trend of student unprofessional or unethical behavior. It is also used to measure institutional success in accomplishing the Quillen College of Medicine’s Professionalism Institutional Educational Objective.
